When adding a new user-defined index, it does not save "create from" settings. 

Steps to reproduce:

1. Create a new User-defined index table.
2. On the first tab "Index/Table", on "Create From", verify "Index marks" is selected.
3. deselect "index marks".
4. Select "tables"
5. Click OK.
6. Edit "Index/Table".
7. "Index marks" is selected and "tables" isn't.

This happens with any setting. It seems it doesn't save your settings only in "create from".

Not reproduced.
Did you try https://wiki.documentfoundation.org/User_Profile#Resolving_corruption_in_the_user_profile
What operating system are you using?

Set to NEEDINFO.
Change back to UNCONFIRMED, if the problem persists. Change to RESOLVED WORKSFORME, if the problem went away.
