Example of index that the last item is at page 10 (for example)

Problem description: 

the index page number are wrong beyond page 10

Steps to reproduce:
1. write 10 pages with subtitles (nested)
2. add page number or an index at the beginning

Current behavior:
Show wrong page number.
show correct index numbers.

Expected behavior:
Correct page number.
              
Operating System: Ubuntu
Version: 4.2.2.1 release
Last worked in: 4.1.5.3 release

Right click TOC - Update index/table fixes the problem on 3.5 and 4.5. Save and reopen retains the TOC numbers just fine.
I'm setting this to INVALID.

Franco: I hope you get the same result. If not, you can reopen this and set to UNCONFIRMED.
